you say you have installed the motherboard chipset drivers- can you check that the agp is enabled in bios and it isnt just looking for some other on board device- I think you said it was dual pci and agp so make sure correct source is enabled - do the same with the sound card make sure it isnt looking for on board sound.
as others have suggested take it out and clean the connections and reseat the card then check and double check all the power supplies on the board and especially the power supply to the card
