The Air France A340 crash at Pearson a few years ago put the blame squarely on the flight crew. The pilot is now suing the Safety Board over it with help from the Pilots' Association. He set the plane down too far down the runway to be able to stop in the poor weather, and the board said it was his decision in the end. That one showed up on "Mayday" on Discovery.

Right click on the taskbar, click properties, click on 'start menu' tab, click on the 'customize' button, select the 'advanced' tab, and select the radio button option that you want for the start menu.
